Eligibility

Who qualifies for support?
Bronx residents who are Medicaid eligible or part of an eligible health plan, and who have a health related social need - food, transportation, care navigation, or preventive support. Final eligibility depends on plan and program review.
Do I need Medicaid to apply?
Most members are reached through Medicaid, but we also serve people on other eligible plans. If you're not sure, call us at (718) 540 0780 and we'll help you check.
Are caregivers eligible to request help?
Yes. A family member, friend, or care advocate can submit a form on behalf of an eligible member, with their consent.
What if I am unsure of my plan?
That's fine. Start the eligibility check anyway. Our team will help you figure it out.

Meal & Grocery Program

How quickly can meals start?
Most members hear back within one business day. Meal and grocery deliveries typically begin within one to two weeks after screening is complete.
Can meals be culturally appropriate?
Yes. Our nutritionist plans menus around cultural preferences and dietary needs, including ingredient choices and preparation styles.
Are meals free for eligible members?
Yes, there is no out of pocket cost for eligible members.
Can a family member receive meals on my behalf?
The meals and groceries are delivered to the member's address. A trusted family member or caregiver can be at home to receive them.
Is service available in Spanish or Bengali?
Yes. Our team supports English, Spanish, and Bengali across phone, email, and in person.

Transportation

What rides are covered?
Non emergency medical transportation - appointments, follow ups, pharmacy pickups, and community resource visits.
How far in advance do I book?
Schedule rides at least 48 hours in advance when possible. Urgent appointment rides may be accommodated sooner.
Can my caregiver come along?
Yes, a caregiver or escort can ride along when needed for the member's safety or comfort.

Care Management

What does a care advocate do?
Your care advocate coordinates appointments, follow ups, referrals, and community resources. They are your single point of contact across the network.
How often will we talk?
It depends on your needs. Some members talk to their advocate weekly, others monthly. We adjust as life changes.
Will my provider be involved?
Yes, with your consent. We coordinate directly with your providers so nothing falls through the cracks.

Provider Referrals

Who can submit a referral?
Any provider, clinic, social worker, care manager, or community partner can submit. The member needs to give consent before we contact them.
What information is required?
Member name, contact, and the service needed. Anything else helps but is optional.
How long until you contact the member?
We reach out within one business day in the member's preferred language.
